Family Assessment & Planning Team

About the Team
The Family Assessment and Planning Team (FAPT)  has monthly meetings involving service planning for eligible children and families by representatives from Department of Social Services, Colonial Behavioral Health, 9th District Court Service Unit, WJC Schools, Peninsula Health District, the community, local private providers, and parents.
